Understanding Leapfrog's hospital information

You can search the Leapfrog hospital information by zip code, city, or state and view each hospital’s progress at implementing the Leapfrog-recommended quality and safety practices by looking at the circles in the chart and comparing them to the explanation on every page. While the Leapfrog survey is voluntary, we also show hospitals that were invited to complete the Leapfrog Hospital Survey but decided not to do so. 

Some of the hospitals that completed the survey may have provided more information regarding their approach to improving the quality of care or other safety initiatives they have in place. We encourage you to review that additional information. Click the "i" next to a hospital's name in the table to visit the hospital's Web site. We also encourage you to contact the hospital directly with your questions and feedback.
