The LHRP evaluates hospital performance across the quality and resource utilization measures captured in the Leapfrog Hospital Survey. All hospitals that wish to participate in the LHRP must submit a completed Leapfrog Hospital Survey according to the specified timeline: June 30 for initial results and December 31 for calendar-year results.

The quality and resource utilization measures are weighted and rolled up into an overall efficiency score which is used to determine recognition and rewards levels. The quality measures account for 65% of the overall efficiency score and the resource utilization measures account for 35% of the overall efficiency score.

The overall efficiency score places hospitals into the Attainment category or the Improvement category. These categories are independent of each other and qualify hospitals for different types of recognition and rewards as determined by the local market payer administrating the LHRP.
