Participation Requirements

To be eligible for LHRP, hospitals must complete and submit a Leapfrog Hospital Survey each year. The Leapfrog Hospital Survey is free for hospitals.

The deadline to submit the Survey is June 30 for the initial results; however, hospitals can submit their Survey after this deadline if the deadline for the local payer administering LHRP is after the initial cutoff date. Surveys must be submitted by December 31 in order to be included in LHRP results and rewards for that calendar year.
