To be eligible for the LHRP, hospitals must complete and submit the required performance data through the Leapfrog Hospital Survey each year. The Leapfrog Hospital Survey is free for hospitals.

The deadline to submit the Survey is June 30 for the initial results; however, hospitals can submit their Survey after this deadline. Surveys must be submitted by December 31 in order to be included in LHRP results and rewards for that calendar year.

Health plan licensees can work with Leapfrog to determine the geography and scope of their LHRP market(s). Licensees can target specific hospitals—by zip code, region, or state—to participate in the Leapfrog Hospital Survey and determine performance benchmarks based on local, state, or national data results.
