Leapfrog asks hospitals to complete the on-line survey through our network of 38 Regional Roll-Outs. CEO’s are sent an introductory letter requesting that your hospital completes the survey. This letter also contains a security code for completing the survey on-line. If you do not have a security code and your hospital is in one of the regions listed above then call your regional contact to find out where the code was sent. They can also arrange to have another copy sent.

Participation in the survey is voluntary. However, urban hospitals that are targeted through the Regional Roll-Outs and choose not to share their information are shown as not taking part. 

Hospitals outside of our regions are also more than welcome to complete the survey. Please complete the following form to obtain an ID and security code.
