How the Leapfrog survey data is gathered

Participation in the Leapfrog Hospital Survey  is voluntary. Leapfrog asks hospitals to complete the on-line survey through a network of 41 business health coalitions, quality improvement organizations, and direct purchasing groups called Regional Roll-Outs. On behalf of Leapfrog, the Regional Roll-Out's send Hospital CEO's an introductory letter requesting that their hospital complete the Survey. This letter accompanied by a security code for completing the Survey on-line. If the hospital does not have a security code and the hospital is in one of the Regional Roll-Out regions (check by clicking on the link), then a representative from that hospital should call the regional contact to find out where the code was sent. Hospitals can also request a security code by emailing the Leapfrog Help Desk at leapfrog@quantros.com.

Although the Survey is voluntary, hospitals targeted through the Regional Roll-Out  process that choose not to share their information are shown as "Declined to Respond" on the Hospital Ratings  section of Leapfrog's website.
