Scoring Algorithm for Never-Events Policy
A hospital’s results are publicly released and displayed on the Leapfrog Group Web site in one of four categories:
Fully Meets Standards (Four filled bars) means the hospital has implemented a policy that adheres to all of the principles of the Leapfrog Group Policy Statement on Serious Reportable Events/ “Never Events” (answered “Yes” to # 1).
Some Progress (Two filled bars) means the hospital has not yet fully implemented such a policy but has committed to implementing the Leapfrog Group Policy Statement on Never Events within 60 days of survey submission and to resubmitting a survey indicating its implementation. (answered “No” to #1 but “Yes” to #2).
NOTE: This partial credit will be provided only for two months following a hospital’s first-time commitment to implement the policy fully. Having made this commitment, a hospital must:

return to the online survey by the end of the second calendar month after submitting a survey with that commitment,

update and affirm responses indicating full implementation of the policy , and
-
resubmit its survey to earn Fully Meets Standards credit.
Otherwise, after that time period, the hospital’s results will be revert to Willing to report . Subsequent re-commitments to implement within 60 days will not earn Some Progress partial credit after that initial two-month time period has passed.
Willing to report (1 filled bar) means the hospital responded to both Leapfrog survey questions pertaining to adoption of this policy, but does not yet meet the criteria for a good early stage effort (answered “No” to #1 and #2).
Declined to Respond means the hospital did not respond to this section of the survey, or the hospital was asked to complete the survey but has not submitted one.
Adherence to Never Events Policy
The Leapfrog Group asks hospitals to agree to all of the following if a never event occurs within their facility:

We will apologize to the patient and/or family affected by the never event

We will report the event to at least one of the following agencies within 10 days of becoming aware that the never event has occurred:

Joint Commission, as part of its Sentinel Events policy*

State reporting program for medical errors

Patient Safety Organization (e.g. Maryland Patient Safety Center)

We agree to perform a root cause analysis, consistent with instructions from the chosen reporting agency

We will waive all costs directly related to a serious reportable adverse event
*Reporting of Sentinel Events to the Joint Commission is voluntary for its accredited hospitals. |
