Survey Login and Materials

The 2025 Leapfrog Hospital Survey is free and open to hospitals from April 1 to November 30. The annual survey assesses hospital safety, quality, and efficiency based on national performance measures that are of specific interest to health care purchasers and consumers. Survey Results are publicly reported, and provide hospitals with information to benchmark their progress in improving the care they deliver. For general, acute-care hospitals only, some Survey Results are also used to calculate the Leapfrog Hospital Safety Grade.

Proposed Changes to the 2026 Leapfrog Hospital Survey

Each year, The Leapfrog Group’s research team reviews the literature and convenes expert panels to ensure the Leapfrog Hospital Survey aligns with the latest science and the public reporting needs of purchasers and consumers. Once the list of proposed changes is assembled for next year’s Survey, Leapfrog releases that list for public comment. The public comments received are then reviewed by Leapfrog’s research team and used to refine the Survey before it is finalized. The public comment period ended on December 18.

Alignment with Other National Programs

Many of the measures included in the Leapfrog Hospital Survey are in use by other national agencies and organizations including the Centers for Medicare & Medicaid Services (CMS), the CDC's National Healthcare Safety Network (NHSN), and The Joint Commission. For general hospitals, some of the measures are used to calculate the Hospital Safety Grade.

Leapfrog's Rating Programs

Leapfrog administers two different ratings programs for hospitals: the Leapfrog Hospital Survey and the Leapfrog Hospital Safety Grade. Most every hospital is targeted to submit an annual Hospital Survey, but only general hospitals are eligible for a Hospital Safety Grade.
