Survey Login and Materials

The 2026 Leapfrog Hospital Survey is free and open to hospitals from April 1 to November 30. The annual survey assesses hospital safety, quality, and efficiency based on national performance measures that are of specific interest to health care purchasers and consumers. Survey Results are publicly reported, and provide hospitals with information to benchmark their progress in improving the care they deliver. For general, acute-care hospitals only, some Survey Results are also used to calculate the Leapfrog Hospital Safety Grade.

2026 Leapfrog Hospital Survey Results are now available for review on the secure Hospital Details Page in advance of being publicly reported on July 25 for hospitals that submitted a Survey by the June 30 Submission Deadline. 

Hospitals are urged to review their Survey Results for accuracy and completeness.

Honoring Hospitals that Choose Transparency

To honor the hospitals and ASCs that have made the commitment to their communities to be transparent, Leapfrog is launching a new “Full Transparency” badge. To be eligible for the Full Transparency badge, hospitals and ASCs must complete, affirm, and submit all sections of the Leapfrog Surveys.

Scoring Algorithm for CMS OAS CAHPS Updated

Leapfrog updated the scoring cut-points for the OAS CAHPS measures based on the most recent CMS data for hospitals and ASCs. Facilities without data in the CMS Provider Data Catalog will be publicly reported as "Not Available".

Alignment with Other National Programs

Many of the measures included in the Leapfrog Hospital Survey are in use by other national agencies and organizations including the Centers for Medicare & Medicaid Services (CMS), the CDC's National Healthcare Safety Network (NHSN), and The Joint Commission. For general hospitals, some of the measures are used to calculate the Hospital Safety Grade.
