ASC Public Reporting Program
Leapfrog scores and publicly reports 13 measures from the CMS Ambulatory Surgery Center Quality Reporting (ASCQR) Program on our Public Reporting website to ensure that patients and family caregivers have a place to easily search for and compare facilities.
ASCs with active accreditation through AAAHC or JC are scored and publicly reported as Achieved the Standard (best score) for 5 measures that previously required additional data collection to report performance via the Leapfrog ASC Survey. Visitors to Leapfrog’s public reporting website will be able to see achievement of these important patient safety standards that ASCs have earned through their rigorous accreditation process.
Leapfrog scores and publicly reports scores on 8 measures not nationally available by any other source, based on submissions of the ASC Survey. Visitors to our public reporting website will be able to see and compare ASC’s progress towards achieving these robust, evidence-based patient safety and quality standards, identify ASCs that have earned Leapfrog’s prestigious Top ASC Award, and quickly identify those ASCs that have demonstrated full transparency through voluntary reporting.
A Vision of Progressive Transparency to Drive Patient Outcomes
The Leapfrog ASC Public Reporting Program gives the public access to meaningful, independent ratings of ASCs to help inform their decisions about where to receive care, while giving ASCs visibility to the employers, purchasers, and health plans they are doing business with.

Submit your ASC Survey 2.0 before the November 30 Final Deadline
ASCs that would like to have results from the 2026 ASC Survey 2.0 publicly available at ratings.leapfroggroup.org should access thir ASC Dashboard to get started and submit by November 30. No new submissions are accepted after that date.
Want to be recognized for earning accreditation? Submit your certificate by November 30.
ASCs with active accreditation through the Accreditation Association for Ambulatory Health Care (AAAHC) or Joint Commission (JC) will be scored and publicly reported as "Achieved the Standard" (best score) for five patient safety measures. ASCs are required to upload proof on AAAHC or JC’s accreditation on the ASC Survey Dashboard.
Honoring Hospitals and ASCs that Choose Transparency
Transparency is one of the most powerful drivers of change in health care. To honor the hospitals and ASCs that have made the commitment to their communities to be transparent, Leapfrog is launching a new “Full Transparency” badge. To be eligible for the Full Transparency badge, hospitals and ASCs must complete, affirm, and submit all sections of the Leapfrog Surveys.
