Spring 2024 Hospital Safety Grade Town Hall Call

February 20, 2024

Hospitals and other stakeholders will be invited to a free Town Hall Call to hear about changes to the measures and scoring methodology, as well as details of the Courtesy Safety Grade Review Period and important deadlines. The Town Hall Call includes a 40-minute presentation and a 20-minute Q&A session.

 

Fireside Chat on Patient Safety with PSQH

Preventable medical errors kill more than 250,000 Americans each year. While the number is staggering, the solutions to preventing errors are known and need to be prioritized.

In this 60-minute fireside chat, Leah Binder, president and CEO of The Leapfrog Group, will discuss the strategies that need to be implemented to help improve patient safety and reduce errors such as “never events.” This includes the recommendations made by The Working Group on Patient Safety, which was established by the President’s Council of Advisors on Science and Technology, and Leapfrog’s recommendations on how to reduce diagnostic errors.

2023 Leapfrog Annual Meeting and Awards Dinner

The 2023 Leapfrog Annual Meeting & Awards Dinner was on December 5, 2023, at the Grand Hyatt in Washington, DC. The meeting theme was “The Next Leaps in Patient Safety” and highlighted strategies to dramatically advance patient safety, from supporting the health care workforce to engaging patients to better use technology. We also spotlighted public policy initiatives that will guide fundamental—and much needed—change. Each year, the Top Hospital Awards and Top Ambulatory Surgery Center (ASC) Awards are presented during the Awards Dinner. The Top Hospital Awards and the Top ASC Awards are widely acknowledged as one of the most competitive awards United States hospitals and ASCs can receive for patient safety and quality.

Case Study in Improving the Safety and Quality of Diagnosis in Hospitals

Dr. Divvy Upadhyay, MD, MPH, leads diagnostic safety efforts at Geisinger in Pennsylvania. He was the Researcher-in-Residence at the Safer Dx Learning Lab and scientist in the Division of Quality, Safety and Patient Experience at Geisinger and will present approaches to driving improved diagnostic safety and quality by sharing lessons learned across different organizations, including Geisinger. Jill Dykstra-Nykanen, RN, MSN, CPHQ, Chief Quality Officer at Orlando Health Arnold Palmer Hospital for Children, will present how her organization has prioritized improving diagnosis across several quality of care interventions.

Become a Recognized Leader in Caring for People with Diabetes- A New Program of The Leapfrog Group and the American Diabetes Association

On November 1, The Leapfrog Group and the American Diabetes Association (ADA) are launching a new national program to recognize hospitals that are providing safe, high-quality care to hospitalized patients with diabetes, regardless of the reason for their hospitalization. International Infection Prevention Experts Discuss Hand Hygiene

Hand hygiene compliance is a key quality measure for infection prevention and control programs, patient safety, and quality of health services worldwide. It is one of the most simple and effective measures to reduce healthcare associated infections (HAIs). However, compliance rates are as low as 6% - 14% when assessed by both the WHO 5 moments and 6-step hand hygiene technique, and as much as 50% of avoidable infections are caused by poor hand hygiene. This session examined how hand hygiene compliance is measured, real-world challenges of hand hygiene, trends and future developments including the new international standard for hand hygiene, and the role technology can play in helping hospitals achieve compliance of hand hygiene standards.

Ambulatory Surgery Center (ASC) Policy Briefing: Advancing Quality, Safety, and Public Reporting 

Join leadership from Leapfrog and Ambulatory Surgery Center Association (ASCA) for an informational webinar on Leapfrog's national efforts to advance patient safety and quality measurement and public reporting in ambulatory surgery centers. Leapfrog's Never Events Policy and the PACT Collaborative Resources and Tools
