Published in 1999, the report sparked a national movement for patient safety and accountability. This conversation brings together leaders in health care quality, policy and patient advocacy to reflect on the strides made, examine persistent challenges and reignite the call to action for a safer, more transparent health care system.

Artificial intelligence is rapidly transforming health care—but what does it mean for patient safety?
