Measures of Utmost Importance to Purchasers and Employers
ASCs are urged to submit the Leapfrog ASC Survey 2.0. Visitors to our public reporting website will be able to see and compare ASC’s progress towards achieving these robust, evidence-based patient safety and quality standards, identify ASCs that have earned Leapfrog’s prestigious Top ASC Award, and quickly identify those ASCs that have demonstrated full transparency through voluntary reporting.
Eligibility
ALL ASCs that have reported data in the CMS provider data catalog (with the exception of those missing data for all four of the ASC 1-4 measures) are eligible to submit an ASC Survey 2.0 and ASCs that are not participating with CMS but have historically submitted the Leapfrog ASC Survey (e.g., Military Treatment Facilities, pediatric surgery centers, etc.).
Eight Leapfrog ASC Survey 2.0 Measures
- Billing Ethics: Transparent billing practices and patient cost communication
- Health Care Equity: Commitment to reducing healthcare disparities and equitable care delivery
- Informed Consent: Informed consent policies, training, and process
- Taking Responsibility for Never Events: Policies and practices for responding to serious preventable adverse events
- Infection Surveillance for ASCs Performing Breast Surgeries, Laminectomies, Herniorrhaphies, or Knee Prosthesis Procedures: NHSN OPC SSI Reporting Plans
- Hand Hygiene: Hand hygiene compliance monitoring and improvement programs
- Culture of Safety: Measuring culture of safety and taking steps to improve on those results, like staff education and performance improvement
- National Volume Standards for Total Knee Replacement, Total Hip Replacement, and Bariatric Surgery for Weight Loss: Annual procedure volumes for high-risk surgical procedures
- Facility Volume for Select Procedures (Optional): Not scored; however, responses are used to facilitate the search functionality on Leapfrog's public reporting website and the information is publicly reported to inform purchasers and consumers about the facility's experience with the procedure.

Step 1: Access the ASC Dashboard
Request your free, secure 16-digit security code to access the ASC Dashboard. This is where you will upload accreditation documentation and/or complete the online ASC Survey 2.0.

Leapfrog ASC Survey Binder
The Leapfrog ASC Survey Binder document (PDF) is available for use by all hospitals to collect, organize, and record information during the completion of the Leapfrog ASC Survey. The document, which is divided up into 5 tabs (or sections), one for each section of the Survey, can be printed and placed in a binder. The information is helpful when completing subsequent years' Surveys, in staff and leadership transitions, and as a historical record.
