Your Path to Participation
Step 1: Access the ASC Dashboard
Request your free, secure 16-digit security code to access the ASC Dashboard. This is where you will upload accreditation documentation and/or submit the ASC Survey 2.0.
Step 2: Choose Your Level of Participation
Minimum (No Action Required):
- Leapfrog will automatically score and publicly report 13 CMS ASCQR measures for your facility if you participate in the CMS ASCQR program
- These includes nine outcome measures, and four patient experience measures: Full List
- Note: Only facilities with at least one rate for ASC 1 through ASC –4 will be reported. Measures that do not apply to your facility because you do not perform a given procedure will not be reported.
- No data submission required from your facility
Enhanced (Accreditation Upload):
- Upload your current AAAHC or Joint Commission accreditation certificate
- Be scored and publicly reported as "Achieved the Standard" for 5 patient safety measures
- Clinicians Certified in Advanced Life Support Always Present,
- Use of a Safe Surgery Checklist,
- Medication and Allergy Documentation,
- Leadership Structures and Systems to Support Patient Safety, and
- Identification and Mitigation of Patient Safety Risks.
- Reduces duplicative reporting burden
- Estimated time: 5 minutes (basic demographic information + certificate upload and affirmation)
Full Transparency (ASC Survey 2.0 Submission):
- Complete and submit the Leapfrog ASC Survey 2.0
- Be scored and publicly reported on 8 additional measures unavailable elsewhere
- Become eligible for Leapfrog's prestigious Top ASC Award designation
- Estimated time: Approximately 10 hours of staff time (a 50% reduction from previous years)
- Note: To be eligible for Top ASC designation, you must upload accreditation AND submit the Survey by August 31

Leapfrog offers several opportunities for ASCs to learn about every aspect of the ASC Public Reporting Program. ASCs are encouraged to contact our Help Desk with any questions regarding the Program, including the ASC Survey 2.0 or to request a 30-minute 1:1 orientation. The Help Desk is staffed from 9:00 a.m. to 5:00 p.m. ET on all regular business days. Help Desk tickets are responded to within 1-2 business days, if not sooner.
