Diagnostic Safety and Quality Webinar Series: Overview and Implications for Hospitals

Leapfrog’s 2024 Hospital Survey will feature new fact-finding questions to assess how hospitals have approached diagnostic safety and quality, including both steps taken to reducing missed or incorrect diagnoses and encouraging the effective communication of diagnoses to patients, their caregivers, and providers. Before the launch, Leapfrog will provide a free webinar series to review what diagnostic errors are and how they affect care in hospitals, describe resources available to hospitals to make an immediate impact on their quality of care, and learn from hospitals that have made tremendous progress on their journey to reduce diagnostic errors. Each webinar will include opportunities for discussion, and a chance to have all your questions answered.

Dr. Hardeep Singh, MD, MPH, a patient safety researcher at Center for Innovations in Quality, Effectiveness and Safety (IQuESt) based at the Michael E. DeBakey VA Medical Center and Baylor College of Medicine, Houston, will describe two new and compelling resources for hospitals looking to get started on the path towards diagnosis: Measure Dx: A Resource To Identify, Analyze, and Learn From Diagnostic Safety Events, and the Safer Dx Checklist, an organizational self-assessment tool with 10 recommended practices to achieve diagnostic excellence. Dr. Divvy Upadhyay, MD, MPH, leads diagnostic safety efforts at Geisinger in Pennsylvania. He was the Researcher-in-Residence at the Safer Dx Learning Lab and scientist in the Division of Quality, Safety and Patient Experience at Geisinger and presents approaches to driving improved diagnostic safety and quality by sharing lessons learned across different organizations, including Geisinger. Jill Dykstra-Nykanen, RN, MSN, CPHQ, Chief Quality Officer at Orlando Health Arnold Palmer Hospital for Children, presenting how her organization has prioritized improving diagnosis across several quality of care interventions. The session includes an open and frank conversation about how participants can leverage learnings from the webinar series to kickstart improvement at their institutions.
